RECA Information Request
Please complete the form below to request Radiation Exposure Compensation Act (RECA) related information from the Ritenour School District. The details you provide will help us verify your connection to the district and ensure you receive accurate information.
Please be sure all information on the form is accurate, including your contact information. Allow 4-6 weeks from the submission of this form for processing of the records requests by the Ritenour School District.

Please note: Before the Ritenour School District can release any records, we will need to see a valid form of photo identification. This can be done in person or electronically via email. If the person whose records are being requested is deceased, we may also require a death certificate or other documentation to confirm your eligibility to receive those records. If you are representing a family member who is unable to make the request themselves, we may also require additional documentation.
